This is the point where all the main characters have been introduced long enough to have developed favorites and trained enough to handle some level of consistent danger so get ready for stuff to start going crazy! Tomura's hands have a meaning but I don't think you learn about that until the end of season 5 or the new season 6. No one reacted to the hand to his throat because of what the episode was discussing everyone assuming there safe and people have morals and won't do bad things, in public places especially.
