I once again found a decent workaround to get this prefab working in UDON. Let's see how long it lasts.

Here's a non-working version of the Discord Feedback Form for UDON (SDK3) worlds. VRChat once again took away another handful of tools from world creators.

Even if it did work, users must enable "Allow Untrusted URLs" in their settings and must use underscores instead of spaces.

Keep in mind VRChat's networking is super wonky so it might look laggy if you don't have ownership of the objects you're using it on.

Vanity Nameplates are Dead

VRChat's latest update, build 928, has added a random offset to all nameplates. This means vanity nameplates won't line up perfectly anymore, so it'll look like the picture above.

It was fun while it lasted guys, but it's time to move on to bigger and better things.
