Kaylee knows River is drifting in and out of sanity. Last episode she randomly (seemingly) attacked Jayne with a knife. Now Kaylee just found out River is r-e-a-l-l-y good at killing people. So yeah, Kaylee is a little scared of River now. I would be too. Middleman: a person or company that buys stuff and the resells it at a higher price. Like when grocery stores buy from farmers and sell to shoppers, the grocery store is the "middleman".

At some level, with the Shan Yu "meet the real you" introduction, this was about Mal pushed to the limit. You mentioned most people would give up and just submit to the torturer. But not Mal. Mal "didn't break", and Mal did whatever he could to protect his crew, even if that meant infuriating Wash so he would not give up and die. This episode also shows how strong and unfazed Zoe is as well. You can see how Mal and Zoe were an amazing team during the Unification War. This episode also showed that River is "bad ass" and not to be underestimated.
