As far as I know, the Alluka's gender thing is never fully explained. All we know is that Killua calls Alluka "she" while the rest of the family calls Alluka "he." I think this implies Alluka is biologically male but wants to be a girl (Killua is the only one who truly cares about her and would respect that), but that's just a guess.
