I don't know who was the first but I know there was a Austrailian actress in the early Hindi industry known as Fearless Nadia in the 30's, so I think it is something that is common from very early on. Telugu films have Super Star Krishna who was a big star in the 70's and is Mahesh Babu's Father. Mahesh is also known as Super Star. So even though there is only one Rajinikanth, there are many "SuperStars" Nani got his name because of his acting style is more 'grounded' compared to what was commonly required of the star Heroes in the early 2000s in Telugu mass films. He is also not as trapped into Mass roles by his fans like the other Heroes are and so can often choose a wider variety of roles. Incidently the Telugu Film Industry started and filmed primarily in Chennai in Tamil Nadu up until the 1990's when it shifted to Hyderabad. Mahesh Babu, Prabhas, Allu Arjun and Ram Charan were all born in Chennai and I believe they are all fluent in Tamil. Jr NTR was not and even though he is technically a 'star kid', he did not grow up with or benefit from his Father's family connections until much later. That said he is also fluent in Tamil.
