The mission statement for this story, and Saint Dorothy's Home itself, is essentially "those other facilities are too nice to their patients; let's REALLY show them how cruel we can be", and honestly? That made for some fun writing.
I even took the opportunity to have our lead Nurse here be a returning Monique from Bad Beat! I hadn't yet determined where she was working exactly when I finished Bad Beat, and was considering having her show up in my next story set at Sunny Skies (as seen in Two Ladies and The Doctor's Wife/Dear Darling Doctor), when the commissioner asked me to develop a third, harsher facility. To help it feel a bit more connected to the rest of the setting, I tossed Monique in there as the Head Nurse!
