I think Great is in a loop, every time he sees a vision, and all the scenes before the intro seem like it's part of a different timeline. It seems like it is past experiences. I need a big whiteboard just like Great.

I agree with you baozibang i think great is in a coma and we are in his brain (sort of ) during the episode and we see the story that lead to the first scene of ep 1 . The scenes before the opening are, like you said, in the present time and the 2 stories are gonna line up at some point for us to understand everything. I think great is relieving his story that’s why he has this flash and somehow when his heart has a failure, he can imagining changing his story a little bit and it is 4 minutes because it’s the time the doctor uses to reanimate him maybe.

It can't be a time loop, or well it's not likely, because we already had Triage and I don't think Sammon would do that twice. I first thought maybe the first scenes are the consequences of the corresponding episodes but that also only makes limited sense and still wouldn't answer anything about the 4minutes ec. (I hate and love that I can't go look it up! I need more original works instead of novel adaptations because as frustrating as this all is, it's SO MUCH FUN!)
