The cruelty of her demands are solely a consequence of how significant the wish is. She isn't actively killing the people that fail the demands either they just explode. She's a slave to both her family and her power.

And after their license getting suspended, now they're not afraid of anything to not vote since they don't have anything to lose anymore. This is one of many things that make this election is interesting, all the mind game by Pariston and others. Since the Zoldycks don’t respect Alluka they continue to call her by her original sex, a boy. Also Ron was right that’s just a general statement. When she calls Killua big brother it’s Alluka. And when she calls him Killua it’s nanika. Those were real requests she made in the room with Killua that’s why he was able to wish for something in order to leave the room.
