Family names are basically like our last names here, so by that logic, every japanese name you hear or see, will always start with their family name. That's why sometimes you have other characters calling them by their family name, reason being that they are talking (formally) instead of straight up calling their first name (informal speech), which is typically used by someone close, or even by being disrespectful. There are a lot of rules when speaking, but this covers most of it.
