Itadori's brain hadn't caught up with the rest of his body dying. The brain can survive for to six to seven minutes after the heart stops. Afterward brain death results when the entire thing, including the brain's stem has irreversibly lost all function. I've remember a case on the ID channel where a son, between late night and early morning, hits his parents hard with an axe, but doesn't know they managed to survive somewhat. The blood-covered dad wakes up, not realizing he has life threatening injuries and starts doing his morning routine (go downstairs, start the coffee maker, try to start breakfast) and dies while picking up the newspaper outside the front door. So, after Sukuna lost control, when Itadori came back he had those six to seven minutes to literally say what was on his mind before his brain began to shutdown causing him to finally die. Hope this helps you understand how he was talking and walking a bit without a heart.
