They're both diapers. 0 is urinary and fecal incontinence. 1 is urinary incontinence that can't be managed by anything less than a diaper. As for caregivers, if you're a soldier out in the field, a change will go much faster if someone else is doing it for you. LI's artwork often mentioned field changes, but he never explained the need for another person to change the wearer.
