Remedial Sex-Ed by SigmaGal, it used to exist on deviantart but idk if its just me but the site's not working. When I searched it now it appeared on e-hentai. I liked the story quite a bit at the time even though it possessed some aspects I didn't care for but the ending was a disappointment for me. Maybe I was just naive and didn't believe it would dare to end in identity death so it blinded me.

I have no particular beef with mental changes as long as the character keeps the memories but if they don't might as well be a different person. This story made me do 2 things I normally don't (reading about established characters in a non canon story and paying for early access), so it's doing something right.
