S4 might expand on this (& honestly the more is shown about Kaguya's life, the more it will become obvious why she is the way she is) but the doctor's first love, he was reminiscing of, was Kaguya's mother Nayotake. Hence why he lamented "what goes around comes around", thinking she had her mother's heart disease too & then later said "Nayotake, your daughter is doing fine" while looking at the moon

Episode 7 is legitimately one of the funniest episodes of anime ever made. These episodes also coincidentally introduce two of the major crossover points with Oshi no Ko. The manga that sends the student council into Shoujo Manga Brain is in fact “Sweet Today” which is what Kana stars in and brings Aqua in as a guest for the final episode. Next is more obscure, but all signs point to Pieyon being the cheerleading captain (they even got the same actor to voice him in the anime).
